In-Home Nurse Practitioner or Physician Assistant Per Diem - Kalamazoo

Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Kalamazoo area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, lasting approximately 45 minutes each. You will work independently using an iPad and intuitive clinical workflows to deliver personalized care without the requirement to prescribe medications, order lab tests, or alter current treatment regimens.

Clinical Setting

Your primary workplace is the private residences of health plan members. As an in-home clinician, you will meet patients where they live in diverse home environments. You must be able to comfortably and safely perform your duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations including medication and medical history review
• Perform physical examinations and assessments
• Administer diagnostic and preventive services as ordered by health plans, including spirometry, diabetic eye exams, and bone density screening
• Provide health education and guidance to help patients better understand and manage their health
• Answer patient health questions and connect patients to appropriate care resources
• Conduct virtual visits as needed
• Travel locally and within licensed states as required
• Work collaboratively with the Signify Health clinical team

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

Compensation and Benefits

• Hourly base rate: $41.85 - $90.13 (actual offer depends on experience, education, geography, and other relevant factors)
