In-Home Nurse Practitioner or Physician Assistant (Per Diem) - Kailua Kona, HI

Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Kailua Kona, Hawaii. In this role, you will visit patients in their homes to conduct comprehensive health assessments, lasting approximately 45 minutes each. You will work independently using an iPad and intuitive clinical workflow to complete evaluations without prescribing medications, ordering lab tests, or altering current treatment regimens.

Clinical Setting

Your primary workplace is the private residences of health plan members. You will meet patients where they live in diverse home environments. You must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations including medication and medical history review
• Perform physical evaluations during each visit
• Administer diagnostic and preventive services as ordered by health plans, including spirometry, diabetic eye exams, and bone density screening
• Provide virtual visits as needed
• Build trusted relationships with patients to support better health outcomes
• Travel locally and within licensed states as needed
• Connect patients to appropriate care resources based on their unique health needs

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

Schedule

• Part-time, per diem position
• Anticipated weekly hours: 12

Compensation and Benefits

• Hourly base rate: $50.22 - $108.15
