In-Home Nurse Practitioner or Physician Assistant Part Time - Newark/Sussex

Signify Health, part of CVS Health, is seeking a part-time Physician Assistant or Nurse Practitioner to provide in-home health evaluations. You will visit patients in their homes to conduct comprehensive health assessments, building trusted relationships and helping bridge gaps in care.

Clinical Setting

As an in-home clinician, your primary workplace is the private residence of health plan members. You will meet patients where they are, in diverse home environments. These environments may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes, allowing time for one-on-one connection and health question answering
• Review medication and medical history with each patient
• Perform physical evaluations
• Provide diagnostic tests as ordered by the patient's health plan, including diabetic eye exams, spirometry, and other diagnostic and preventive services
• Use an iPad and intuitive clinical workflow for each evaluation
• Work as a clinician colleague within the Signify Health network
• Conduct virtual visits as needed
• Travel locally and within licensed states as needed

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted license in coverage area(s)
• MD or DO degree

Preferred Qualifications

• Multi-state licenses

• Part-time position
• Anticipated weekly hours: 29.5

• Hourly rate: $125.00
