In-Home Nurse Practitioner or Physician Assistant (Part Time) - Kauai, HI

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations on the island of Kauai, Hawaii. In this role, you will visit patients in their homes to conduct comprehensive health assessments, lasting approximately 45 minutes each. You will work independently using an iPad-based clinical workflow to deliver personalized, preventive health services directly to patients' residences.

Clinical Setting

Your primary workplace is the private residences of health plan members. You will meet patients where they live and provide care in diverse home environments. These settings may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied conditions.

Responsibilities

• Conduct in-home health evaluations including medication and medical history review
• Perform physical examinations using an intuitive clinical workflow on an iPad
• Provide diagnostic and preventive services as ordered by the patient's health plan, including spirometry, diabetic eye exams, and bone density screening
• Build trusted relationships with patients to support their health journey
• Answer patient health questions and provide health education
• Conduct virtual visits as needed
• Travel locally and within licensed states as required
• Work collaboratively with the Signify Health clinical team

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification as a Nurse Practitioner or Physician Assistant

Preferred Qualifications

• Multi-state licenses

Compensation and Benefits

• Hourly pay range: $50.22 - $108.15 (base rate; actual offer depends on experience, education, geography, and other relevant factors)
