In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Seattle/ Pierce, WA

Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Seattle/Pierce, Washington area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, helping bridge gaps in care and connect individuals with appropriate resources for their unique health needs.

Clinical Setting

Your primary workplace is the private residence of health plan members. As an in-home clinician, you will work in diverse home environments and must be able to comfortably and safely perform your duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes, allowing time for one-on-one connection and to answer health questions
• Review medication and medical history with each patient
• Perform physical evaluations
• Administer additional diagnostic tests as ordered by the patient's health plan, such as diabetic eye exams, spirometry, and bone density screening
• Use an iPad and intuitive clinical workflow for each evaluation
• Travel locally and within licensed states as needed
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated, including Diagnostic and Preventive Services products

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted license in Seattle/Pierce coverage area
• Board certification

Preferred Qualifications

• Multi-state licenses

Schedule

• Part-time position
• Anticipated weekly hours: 12

Compensation

• Hourly base rate plus additional incentive structure (per visit completed and other incentives as applicable)
• Pay range: $46.03 - $99.14 per hour
• Actual base salary offer depends on experience, education, geography, and other relevant factors
