In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Seattle/ Pierce, WA

Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Seattle/Pierce, Washington area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, lasting approximately 45 minutes each. You will work independently using an iPad and intuitive clinical workflow to complete evaluations that include medication and medical history review, physical examination, and diagnostic testing as ordered by the patient's health plan (such as diabetic eye exams or spirometry). You are not required to prescribe medication, order lab tests, or alter patients' current treatment regimens.

Clinical Setting

Your primary workplace is the private residences of health plan members. You will meet patients where they live in diverse home environments. You must be able to comfortably and safely perform your duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations and physical examinations
• Review medication and medical history with patients
• Perform diagnostic and preventive services as ordered, including spirometry, diabetic eye exams, and bone density screening
• Provide health education and answer patient questions
• Use iPad-based clinical workflow documentation
• Travel locally and within licensed states as needed
• Conduct virtual visits as needed
• Work collaboratively with the Signify Health clinical team

Required Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license in Washington (Seattle/Pierce coverage area)
• Board certification

Preferred Qualifications

• Multi-state licenses

Schedule

• Part-time, per diem position
• Anticipated 12 hours per week

Compensation

• Hourly base rate: $46.03 - $99.14
• Additional incentive structure based on visits completed and other incentives as applicable
• Actual base salary offer depends on experience, education, geography, and other relevant factors
