In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Seattle/ King, WA

Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Seattle/King County, Washington area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, lasting approximately 45 minutes each. You will engage one-on-one with patients, review medications and medical history, perform physical evaluations, and administer diagnostic tests as ordered by the patient's health plan, such as diabetic eye exams, spirometry, and bone density screening. You will use an iPad with an intuitive clinical workflow to document findings and are not required to prescribe medications, order lab tests, or alter patients' current treatment regimens.

Clinical Setting

Your primary workplace is the private residences of health plan members. As an in-home clinician, you will meet patients where they are and provide care in diverse home environments. You must be able to comfortably and safely perform your duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations including medication and medical history review, physical examination, and diagnostic testing
• Build trusted relationships with patients to support their health journey
• Use clinical workflow technology to document and manage patient information
• Provide all Signify Health services including Diagnostic and Preventive Services products
• Travel locally and within licensed states as needed
• Conduct virtual visits as needed
• Work collaboratively as a clinician colleague within the Signify Health network

Required Qualifications

• Active, unrestricted license in Washington (Seattle/King County coverage area)
• Board certification as a Nurse Practitioner or Physician Assistant

Preferred Qualifications

• Multi-state licenses

Schedule

• Part-time, per diem position
• Anticipated weekly hours: 12

Compensation and Benefits

Per diem employees receive an hourly base rate plus additional incentive structure based on visits completed and other applicable incentives. The typical pay range for this role is $46.03 to $99.14 per hour. Actual base salary will depend on experience, education, geography, and other relevant factors.

This part-time position is eligible for a range of benefits including malpractice insurance coverage during Signify Health-related activities, dental, vision, wellness resources, employee discounts, access to certain voluntary benefits, and other programs. Specific benefits depend on hours worked and eligibility.

Travel Requirements

You must be flexible to travel locally and within licensed states as needed to conduct patient visits.
