In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Pullman, WA

Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Pullman, WA. In this role, you will visit patients in their homes to conduct comprehensive health assessments, lasting approximately 45 minutes each. You'll work independently using an iPad-based clinical workflow to connect with patients, review their medications and medical history, perform physical evaluations, and administer diagnostic tests as ordered by their health plan, such as diabetic eye exams, spirometry, and bone density screening. You are not required to prescribe medications, order lab tests, or alter patients' current treatment regimens.

Clinical Setting

Your primary workplace is the private residences of health plan members. You will meet patients where they are, in diverse home environments that may include household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations including medication and medical history reviews
• Perform physical examinations and administer diagnostic and preventive services
• Provide health education and answer patient questions during visits
• Use iPad-based clinical workflow to document and manage patient information
• Travel locally and within licensed states as needed
• Conduct virtual visits as needed
• Work collaboratively with the Signify Health clinical team

Required Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license in Pullman or Asotin coverage areas
• Board certification

Preferred Qualifications

• Multi-state licenses

Schedule

• Per diem, part-time position
• Anticipated 12 hours per week

Compensation and Benefits

• Hourly base rate of $46.03 to $99.14, plus additional incentive structure based on visits completed and other incentives
