In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Pullman, WA

Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Pullman, WA. In this role, you will visit patients in their homes to conduct comprehensive health assessments, connecting people with appropriate care resources and helping bridge gaps in their healthcare journey.

Clinical Setting

You will work primarily in patients' private residences, meeting health plan members where they live. As an in-home clinician, you will encounter diverse home environments that may include household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations using an iPad and intuitive clinical workflow
• Provide diagnostic and preventive services as ordered by the patient's health plan, including spirometry, diabetic eye exams, and bone density screening
• Answer patient health questions and provide health education
• Conduct virtual visits as needed
• Travel locally and within licensed states as required
• Work collaboratively with Signify Health clinical colleagues

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license in Pullman/Asotin coverage area
• Board certification

Preferred Qualifications

• Multi-state licenses

Schedule

• Part-time, per diem position
• Anticipated 12 hours per week

Compensation and Benefits

• Hourly base rate: $46.03 - $99.14 (actual offer depends on experience, education, geography, and other relevant factors)
