In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Fort Collins, CO

Signify Health, part of CVS Health, is seeking a Per Diem Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Fort Collins, Colorado area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, helping bridge gaps in care and connect individuals with appropriate resources for their unique health needs.

Clinical Setting

Your primary workplace is the private residence of health plan members. As an in-home clinician, you will work in diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations using an iPad and intuitive clinical workflow
• Provide diagnostic and preventive services as ordered by the health plan, including spirometry, diabetic eye exams, and other tests
• Conduct virtual visits as needed
• Answer patient health questions and provide health education
• Travel locally and within licensed states as needed to visit patients
• Work collaboratively with Signify Health clinical colleagues

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license in coverage area(s)
• Board certification
• Ability to cover primary locations in Fort Collins MSA and Larimer County

Preferred Qualifications

• Multi-state licenses

Schedule

• Part-time, per diem position
• Anticipated weekly hours: 12

Compensation and Benefits

• Hourly base rate: $41.85 - $90.13 (actual offer depends on experience, education, geography, and other relevant factors)
