In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Spokane, WA

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Spokane, WA. In this role, you will visit patients in their homes to conduct comprehensive health assessments, building trusted relationships and helping bridge gaps in care.

Clinical Setting

Your primary workplace is the private residence of health plan members. As an in-home clinician, you will meet patients where they are, in diverse home environments. You must be able to comfortably and safely perform your duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes, allowing time for one-on-one connection and answering health questions
• Review medication and medical history with each patient
• Perform physical evaluations
• Provide additional diagnostic tests as ordered by the patient's health plan, including diabetic eye exams, spirometry, and other Diagnostic & Preventive Services products
• Use an iPad and intuitive clinical workflow for each evaluation
• Conduct virtual visits as needed
• Travel locally and within licensed states as needed
• Work collaboratively with Signify Health as a clinician colleague

Note on Clinical Scope

You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted license in Spokane, WA
• Board certification

Preferred Qualifications

• Multi-state licenses

• Anticipated weekly hours: 29.5

Compensation

• Hourly pay range: $46.03 - $99.14
• Actual base salary offer will depend on experience, education, geography, and other relevant factors
