In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Spokane, WA

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Spokane, WA. You will visit patients in their homes to conduct comprehensive health assessments, build trusted relationships, and help bridge gaps in care.

Clinical Setting

As an in-home clinician, your primary workplace is the private residence of health plan members. You will meet patients where they are, in diverse home environments. These environments may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations
• Administer diagnostic tests as ordered by the patient's health plan, including diabetic eye exams, spirometry, and bone density screening
• Use an iPad and intuitive clinical workflow for each evaluation
• Provide one-on-one patient engagement and answer health questions
• Conduct virtual visits as needed
• Travel locally and within licensed states as needed
• Work collaboratively with Signify Health as a clinician colleague

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted license in Spokane, WA
• Board certification

Preferred Qualifications

• Multi-state licenses

• Part-time position
• Anticipated weekly hours: 29.5

Compensation

• Hourly pay range: $46.03 - $99.14
• Actual base salary offer depends on experience, education, geography, and other relevant factors
