In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Seattle/ Pierce, WA

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Seattle/Pierce, Washington area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, helping bridge gaps in care and connect individuals with appropriate resources for their unique health needs.

Clinical Setting

You will work primarily in patients' private residences, meeting health plan members where they are. As an in-home clinician, you will encounter diverse home environments that may include household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations
• Administer diagnostic tests as ordered by the patient's health plan, including diabetic eye exams, spirometry, bone density screening, and other preventive services
• Use an iPad and intuitive clinical workflow to document evaluations
• Engage one-on-one with patients to answer health questions and provide education
• Conduct virtual visits as needed
• Travel locally and within licensed states as required
• Work collaboratively with Signify Health as a clinician colleague

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license in Seattle/Pierce coverage area
• Board certification

Preferred Qualifications

• Multi-state licenses

Compensation and Benefits

• Pay range: $46.03 - $99.14 per hour (base hourly rate; actual offer depends on experience, education, geography, and other relevant factors)
