In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Poughkeepsie, NY

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Poughkeepsie, NY area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, helping bridge gaps in care and connect individuals with appropriate resources for their unique health needs.

Clinical Setting

As an in-home clinician, your primary workplace is the private residence of health plan members. You will meet patients where they are, in diverse home environments. These settings may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied conditions.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations
• Administer diagnostic tests as ordered by the patient's health plan, such as diabetic eye exams and spirometry
• Use an iPad and intuitive clinical workflow to document evaluations
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products
• Conduct virtual visits as needed
• Travel locally and within licensed states as needed
• Build trusted relationships to support patients' health goals

Key Role Features

You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens. Each visit allows time to connect one-on-one with patients and answer their health questions. You will work as a clinician colleague within Signify Health's national network of purpose-driven clinicians.

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification
• Ability to cover Poughkeepsie and surrounding area

Preferred Qualifications

• Multi-state licenses

Compensation and Benefits

Pay Range: $46.03 - $99.14 per hour

The actual base salary offer will depend on experience, education, geography, and other relevant factors.
