In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) Portland ME

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Portland, Maine area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, helping bridge gaps in care and connect individuals with appropriate resources for their unique health needs.

Clinical Setting

You will work as part of a national network of clinicians providing in-home preventive and diagnostic services. Visits take place in patients' homes and last approximately 45 minutes, allowing for one-on-one engagement and thorough health assessment.

Responsibilities

• Conduct in-home health evaluations using an iPad and intuitive clinical workflow
• Review medication and medical history with each patient
• Perform physical evaluations
• Administer diagnostic tests as ordered by the patient's health plan, including diabetic eye exams, spirometry, bone density screening, and other preventive services
• Answer patient health questions and provide health education
• Conduct virtual visits as needed
• Travel locally and within licensed states as required
• Build trusted relationships to support improved health outcomes

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license in coverage area(s)
• Board certification
• Ability to cover primary locations in Penobscot, Maine

Preferred Qualifications

• Multi-state licenses

Compensation and Benefits

Pay Range: $37.66 - $81.11 per hour (base rate; actual offer depends on experience, education, geography, and other relevant factors)
