In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Penobscot, ME

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Penobscot, ME area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, connecting people with appropriate care resources and helping bridge gaps in their healthcare journey.

Clinical Setting

Your primary workplace is the private residence of health plan members. As an in-home health clinician, you will work in diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations using an iPad and intuitive clinical workflow
• Provide diagnostic and preventive services as ordered by the patient's health plan, including spirometry, diabetic eye exams, and bone density screening
• Answer patient health questions and provide health education
• Conduct virtual visits as needed
• Travel locally and within licensed states as required
• Build trusted relationships to support improved health outcomes

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

Compensation and Benefits

Pay Range: $37.66 - $81.11 per hour

The actual base salary offer will depend on experience, education, geography, and other relevant factors.
