In-Home Health - Nurse Practitioner or Physician Assistant (Part-time) Lebanon NH

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Lebanon, NH area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, engage in one-on-one clinical interactions, and help connect individuals with appropriate care resources.

Clinical Setting

Your primary workplace is the private residence of health plan members. You will meet patients where they are, in diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations using an intuitive clinical workflow on an iPad
• Provide diagnostic and preventive services as ordered by the patient's health plan, including spirometry, diabetic eye exams, and bone density screening
• Answer patient health questions and provide health education
• Conduct virtual visits as needed
• Travel locally and within licensed states as required
• Work collaboratively with Signify Health clinical colleagues

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

Compensation and Benefits

• Pay range: $41.85 - $90.13 per hour (base rate; actual offer depends on experience, education, geography, and other relevant factors)
