In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) -Kennewick/ Walla, WA

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Kennewick and Walla Walla, Washington area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, lasting approximately 45 minutes each. You will build trusted relationships with patients while helping bridge gaps in their care and connecting them to appropriate resources.

Clinical Setting

As an in-home clinician, your primary workplace is the private residences of health plan members. You will meet patients where they are, in diverse home environments. These environments may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations including medication and medical history review, physical examination, and diagnostic testing when ordered by the health plan
• Provide diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening as indicated
• Use an iPad and intuitive clinical workflow to document evaluations
• Engage one-on-one with patients to answer health questions and provide education
• Travel locally and within licensed states as needed to visit patients
• Conduct virtual visits as needed
• Work collaboratively with Signify Health as a clinician colleague

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license in Washington (Kennewick/Walla Walla coverage area)
• Board certification

Preferred Qualifications

• Multi-state licenses

Compensation and Benefits

• Pay range: $46.03 - $99.14 per hour (base hourly rate; actual offer depends on experience, education, geography, and other relevant factors)
