In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Bridgeport, CT

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations. You will visit patients in their homes to conduct comprehensive health assessments, engage in one-on-one clinical interactions, and help connect individuals to appropriate care resources.

Clinical Setting

Your primary workplace is the private residence of health plan members. You will meet patients where they live in diverse home environments. These settings may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied conditions.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations
• Administer diagnostic tests as ordered by the patient's health plan, including diabetic eye exams, spirometry, and bone density screening
• Use an iPad and intuitive clinical workflow to document evaluations
• Conduct virtual visits as needed
• Travel locally and within licensed states as needed to visit patients
• Build trusted relationships to support improved health outcomes
• Connect patients to appropriate care for their unique needs

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

• Part-time position
• Anticipated weekly hours: 29.5

Compensation and Benefits

• Hourly pay range: $46.03 - $99.14 (base rate; actual offer depends on experience, education, geography, and other relevant factors)
