In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Bridgeport, CT

Signify Health, part of CVS Health, is seeking a part-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations. You will visit patients in their homes to conduct comprehensive health assessments, engage in medication and medical history reviews, perform physical evaluations, and administer diagnostic tests as ordered by health plans. Each visit lasts approximately 45 minutes and allows time for one-on-one patient engagement and health education.

Clinical Setting

As an in-home clinician, your primary workplace is the private residences of health plan members. You will meet patients where they are, in diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations using an iPad and intuitive clinical workflow
• Review medications and medical history with patients
• Perform physical evaluations
• Administer diagnostic and preventive services as ordered, including spirometry, diabetic eye exams, and bone density screening
• Provide health education and answer patient health questions
• Conduct virtual visits as needed
• Travel locally and within licensed states as required
• Build trusted relationships to help patients access appropriate care

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

• Part-time position
• Anticipated weekly hours: 29.5

Compensation and Benefits

• Hourly pay range: $46.03 - $99.14 (base rate; actual offer depends on experience, education, geography, and other relevant factors)
