In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Rockingham/Strafford County

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Rockingham and Strafford County. In this role, you will visit patients in their homes to conduct comprehensive health assessments, lasting approximately 45 minutes each. You will engage one-on-one with patients, review medications and medical history, perform physical evaluations, and administer diagnostic tests as ordered by the patient's health plan, such as diabetic eye exams or spirometry. You will use an iPad with an intuitive clinical workflow to document findings. This position does not require you to prescribe medications, order lab tests, or alter patients' current treatment regimens.

Responsibilities

• Conduct in-home health evaluations with patients in their private residences
• Review medication and medical history with each patient
• Perform physical examinations
• Administer diagnostic and preventive services as ordered, including spirometry and diabetic eye exams
• Use iPad-based clinical workflow documentation
• Conduct virtual visits as needed
• Travel locally and within licensed states as required
• Build trusted relationships to connect patients with appropriate care resources

Clinical Setting

As an in-home clinician, your primary workplace is the private residences of health plan members. Work environments are diverse and may include households with pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied home settings.

Schedule

• Full-time position
• 40 anticipated weekly hours

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

Compensation

The typical pay range for this role is $87,035.00 to $187,460.00 annually. Actual base salary will depend on experience, education, geography, and other relevant factors.
