In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) Portland ME

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in the Portland, ME area. In this role, you will visit patients in their homes to conduct comprehensive health assessments, building trusted relationships and helping bridge gaps in care.

Clinical Setting

You will work as part of a national network of clinicians providing in-home health services. Visits take approximately 45 minutes and occur in patients' homes, with the flexibility to conduct virtual visits as needed. You will serve health plan members across licensed states, with primary coverage in the Portland, ME area.

Responsibilities

• Conduct in-home health evaluations including medication and medical history review, physical examination, and diagnostic testing when ordered by the health plan
• Use an iPad and intuitive clinical workflow to document and manage each evaluation
• Provide diagnostic and preventive services including spirometry, diabetic eye exams, bone density screening, and other ordered tests
• Connect patients with appropriate care resources based on their individual health needs
• Conduct virtual visits as needed
• Travel locally and within licensed states as required
• Deliver all Signify Health services as indicated

Required Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

• Full-time position
• 40 anticipated weekly hours

Compensation and Benefits

• Base salary range: $78,331.00 - $168,714.00 annually
