In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) -New London, CT

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations. You will visit patients in their homes to conduct comprehensive health assessments, engage in one-on-one clinical interactions, and help bridge gaps in care within their communities.

Clinical Setting

As an in-home clinician, your primary workplace is the private residence of health plan members. You will meet patients where they are, in diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations
• Administer diagnostic tests as ordered by the patient's health plan, including diabetic eye exams, spirometry, and bone density screening
• Use an iPad and intuitive clinical workflow to document evaluations
• Provide education to help patients better understand and manage their health
• Conduct virtual visits as needed
• Travel locally and within licensed states as needed to visit patients
• Build trusted relationships to support improved health outcomes

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

Compensation and Benefits

• Base salary range: $95,738.00 - $206,206.00 annually
