The opportunity

Job Description

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations in Kings County, NY. In this role, you will visit patients in their homes to conduct comprehensive health assessments, building trusted relationships and helping bridge gaps in care.

Clinical Setting

You will work primarily in patients' private residences, meeting health plan members where they are. As an in-home clinician, you will encounter diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied settings.

Responsibilities

  • Conduct in-home health evaluations lasting approximately 45 minutes per visit
  • Review medication and medical history with each patient
  • Perform physical evaluations
  • Administer diagnostic tests as ordered by the patient's health plan, including diabetic eye exams, spirometry, bone density screening, and other Diagnostic & Preventive Services products
  • Use an iPad and intuitive clinical workflow to document evaluations
  • Engage in one-on-one patient interactions to answer health questions and provide education
  • Connect patients to appropriate care resources based on their unique needs
  • Conduct virtual visits as needed
  • Travel locally and within licensed states as required

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Compensation and Benefits

The typical pay range for this role is $95,738.00 to $206,206.00 annually. Actual base salary will depend on experience, education, geography, and other relevant factors.

Travel Requirements
