In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Hartford, CT

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations. You will visit patients in their homes to conduct comprehensive health assessments, build trusted relationships, and help bridge gaps in care.

Clinical Setting

You will work primarily in patients' private residences, meeting health plan members where they are. Work environments are diverse and may include households with pets, secondhand smoke, and other potential airborne allergens or irritants. You must be able to comfortably and safely perform your duties in these varied home settings.

Responsibilities

• Conduct in-home health evaluations lasting approximately 45 minutes per visit
• Review medication and medical history with each patient
• Perform physical evaluations
• Administer diagnostic tests as ordered by the patient's health plan, including diabetic eye exams, spirometry, and bone density screening
• Use an iPad and intuitive clinical workflow to document evaluations
• Provide patient education and answer health questions
• Conduct virtual visits as needed
• Travel locally and within licensed states as needed
• Work collaboratively with Signify Health clinical colleagues

Note: You are not required to prescribe medicine, order lab tests, or alter patients' current treatment regimens.

Required Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Board certification

Preferred Qualifications

• Multi-state licenses

• 40 anticipated weekly hours

Compensation and Benefits

• Base salary range: $95,738.00 - $206,206.00 annually
