The opportunity

Job Description

A Path of Care Hospice and Home Health in Weatherford, OK is seeking a full-time RN Case Manager to provide nursing care to patients in their homes and support them throughout their hospice journey. This role combines direct patient care, care coordination, and team leadership in a mission-driven hospice setting.

Responsibilities

  • Complete initial assessments within 48 hours of hospice election
  • Work with the attending physician and interdisciplinary team to complete comprehensive assessments
  • Develop, coordinate, and initiate individualized plans of care
  • Conduct ongoing assessments and update care plans based on changes in patient condition
  • Address physical, psychological, social, and spiritual needs of patients
  • Communicate regularly with physicians and the care team to maintain continuity of care
  • Maintain accurate, timely documentation to meet regulatory requirements
  • Provide leadership by instructing caregivers and volunteers and supervising LPNs and hospice aides
  • Practice infection control and report any signs of abuse
  • Provide direct patient care as needed and assist with discharge planning
  • Participate in professional development, community programs, and agency initiatives

Required Qualifications

  • Active Oklahoma Registered Nurse license
  • At least 1 year of clinical experience
  • Reliable transportation and vehicle liability insurance
  • Strong interpersonal and communication skills
  • Ability to adapt to changing circumstances, mentor others, and work with compassion and patience
  • Commitment to integrity, critical thinking, and teamwork

Schedule

  • Full-time
  • Monday through Friday, 8:00 AM to 5:00 PM
