Hospice Registered Nurse Weekends RN

This position provides on-call hospice nursing support for patients and families in home and nursing facility settings. As an on-call RN, you will coordinate care, manage symptoms and medications, support admissions and discharges, arrange durable medical equipment, and facilitate communication between the hospice interdisciplinary team and care facilities. You will respond to patient and family needs during off-hours to ensure comfort and proper symptom management.

Schedule

This is a flexible, full-time or part-time per diem position with an on-call weekend schedule running Friday 5:00 PM through Monday 8:00 AM. You will not carry phones; a triage nurse handles incoming calls and dispatches you only when a visit is needed.

Responsibilities

• Provide on-call support after agency hours for hospice patients and families
• Coordinate comfort care and symptom management
• Manage medications and durable medical equipment needs
• Support patient admissions and discharges
• Ensure proper documentation of all care provided
• Coordinate care between the hospice interdisciplinary team and nursing facilities

Required Qualifications

• Graduate of an accredited school of nursing
• Current licensure by the Ohio State Board of Nursing
• CPR certification
• Valid driver's license and current driver's insurance coverage
• Self-starter with excellent time management skills
• Ability to multitask in a fast-paced environment
• Dependable and autonomous worker
• Current clinical skills
