Hospice Registered Nurse prn

We are seeking a Hospice Registered Nurse (PRN) to provide compassionate end-of-life care to patients and families in their homes and communities. In this role, you will plan, organize, and direct hospice services, utilizing your nursing expertise and community health knowledge to meet the physical, emotional, and palliative care needs of individuals and families.

Responsibilities

Patient Care

• Complete initial and ongoing comprehensive assessments of patients and families to determine hospice needs, including full physical assessments and medical histories
• Provide professional nursing care using all elements of the nursing process
• Write, initiate, and regularly revise plans of care based on patient status and changing needs
• Use health assessment data to develop nursing diagnoses and establish care goals incorporating palliative nursing actions
• Include patients and families in the care planning process
• Initiate preventive and rehabilitative nursing procedures as appropriate
• Administer medications and treatments as prescribed by the physician
• Counsel patients and families in meeting nursing and related needs
• Provide health care instructions appropriate to patient assessment and plan of care
• Assist patients with activities of daily living while facilitating self-sufficiency and comfort care
• Serve as Case Manager when assigned, coordinating patient care for assigned caseload

Communication and Coordination

• Complete, maintain, and submit accurate clinical notes regarding patient condition and care provided, including pain and symptom management outcomes
• Communicate with physicians regarding patient needs and report changes in condition; obtain and receive physician orders as required
• Coordinate care with community health professionals
• Serve as a mandatory reporter and report suspected abandonment, abuse, financial exploitation, or neglect of vulnerable adults to appropriate agencies immediately when reasonable cause exists
• Report failure to comply with Department of Health requirements within 14 calendar days using required forms
• Teach patients and families self-care techniques, medication management, diet instructions, and other physician-ordered guidance
• Work collaboratively with the interdisciplinary hospice team to meet emotional needs of patients and families
• Attend interdisciplinary team meetings
• Maintain a safe environment for patients
• Assist patients, families, and team members in providing continuity of care

Additional Duties

• Participate in on-call duties as defined by organizational policy
• Ensure arrangements for equipment and necessary services are available
• Supervise ancillary personnel and delegate responsibilities as required
• Maintain professional knowledge and practice skills through continuing education and inservice participation
• Fulfill assigned case responsibilities
• Actively participate in quality assessment and performance improvement activities

Required Qualifications

• Graduate of an accredited school of nursing
• Active Registered Nurse license in good standing
• Minimum one year of professional nursing experience within the last three years, OR a baccalaureate degree in nursing with minimum two years of experience
• Excellent observation, verbal, and written communication skills
• Problem-solving and basic math skills
• Nursing skills per competency checklist
• Ability to perform prolonged or considerable walking or standing
• Ability to lift, position, and transfer patients
• Ability to lift supplies and equipment
• Considerable reaching, stooping, bending, kneeling, and crouching
• Visual acuity and hearing sufficient to perform required nursing skills
• Valid driver's license with personal automobile insured in accordance with state and organizational requirements and in good working order
• Current CPR certification (hands-on demonstration with certified trainer required; internet-based certification not acceptable unless skills were demonstrated hands-on and observed by certified trainer)

Preferred Qualifications

• Minimum two years of professional nursing experience with at least one year in public health, home care, or hospice nursing
