KeystoneCare is seeking a Hospice Registered Nurse (RN Case Manager) to provide compassionate end-of-life care in the Philadelphia County area. In this role, you will work with a Medicare-Certified and CHAP-accredited hospice agency serving the greater Philadelphia region, caring for patients in their homes and hospice settings. You will be part of an interdisciplinary team dedicated to ensuring every patient dies with dignity, free from pain, and supported by compassionate care.
Responsibilities
- Determine the scope and frequency of services needed based on patient acuity and family needs
- Assess and manage patient care needs related to signs and symptoms management, disease progression, patient education, and pain and symptom management
- Promote maximum patient quality of life within patient limitations
- Maximize patient and family compliance with the plan of care
- Coordinate care across the interdisciplinary team for comprehensive, efficient patient care
- Establish plans of care for CNA/HHA disciplines and perform ongoing supervision as required by agency policy
- Maintain current knowledge of the nursing profession through participation in educational programs, conferences, and in-services
- Document care using appropriate terminology, reflecting treatment provided, patient response, results, and pain management
- Submit documentation in a timely manner
Required Qualifications
- Graduate of an accredited school of nursing
- Current RN license in good standing to practice in the Commonwealth of Pennsylvania
- Minimum of one year of experience in homecare or a medical/surgical setting
- Experience with pain and symptom management interventions
- Understanding of hospice philosophy and principles of death and dying
- CPR certification
- Valid driver's license and current car insurance
- Reliable personal vehicle for travel between patients
- Ability to follow oral and written instructions
- Ability to communicate effectively with all members of the care team
- Good organizational skills
- Ability to maintain clinical and professional competence
