Hospice Registered Nurse On Call (RN Weekend days)

Ascend Hospice & Palliative Care KS is hiring a Registered Nurse for on-call weekend day shifts in Topeka, Kansas. This role provides direct hospice nursing care to patients and families during the end-of-life stage, with a focus on pain control, symptom management, and palliative support.

Responsibilities

• Assume responsibility for patient visits and on-call schedules, including assessment, planning, implementation, and evaluation of the nursing process
• Provide hospice nursing care based on systematic assessment focusing on pain control and symptom management
• Complete initial assessments of patients and families to determine home care needs
• Develop and implement individualized care plans based on assessment data
• Anticipate end-stage disease processes and manage related pain and symptoms
• Regularly re-evaluate patient nursing needs and adjust care plans accordingly
• Initiate appropriate preventive and palliative nursing procedures
• Administer medications in accordance with federal and state laws and agency policies
• Counsel patients and families in meeting end-of-life needs
• Perform comprehensive death notes when appropriate
• Maintain current, accurate patient records and documentation at the point of service in accordance with agency policy and Medicare regulations
• Communicate with attending physicians regarding patient needs and report changes in patient condition
• Collaborate with the Medical Director to determine initial and ongoing hospice eligibility
• Coordinate with the interdisciplinary team to develop and implement care plans
• Initiate communication with physicians, hospice staff, and other agencies as needed to coordinate care and resources
• Maintain regular communication with direct supervisor to review assignments
• Ensure arrangements for equipment and other necessary items and services
• Participate in on-call duties as defined by agency policy

Required Qualifications

• State-certified RN license
• Experience in hospice or home health care (required)
• Associate's degree in nursing
• Excellent documentation skills
• Ability to work independently
• Strong communication skills
• Valid driver's license and reliable transportation
• Must be available to travel locally within assigned territory
