Hospice - Registered Nurse - Case Manager - Milwaukee

This position offers meaningful hospice nursing work with a strong emphasis on work-life balance. As a Hospice RN Case Manager, you will provide direct patient care and case management services to patients in Milwaukee County, serving an average caseload of 15-18 patients with 5-6 visits per day.

Clinical Setting

You will work in hospice care, supporting patients and families during end-of-life care. Prior hospice experience is not required.

Schedule

• Monday through Friday, 40 hours per week
• No routine on-call or weekend requirements
• Limited on-call coverage only for vacation and sick leave
• Dedicated after-hours on-call team handles urgent needs

Responsibilities

• Manage a caseload of 15-18 patients
• Conduct 5-6 patient visits per day
• Provide direct patient care and case management
• Collaborate with the interdisciplinary team

Compensation and Benefits

• Up to $45 per hour
• $10,000 sign-on bonus
• Company vehicle provided
• Service area limited to Milwaukee County
