PruittHealth Hospice is seeking a Nurse Practitioner to provide specialized hospice care in patients' homes and care facilities. In this PRN role, you will conduct comprehensive assessments of adult and geriatric patients, evaluate and manage plans of care, and maintain continuity of care within a hospice setting.

Responsibilities

  • Conduct comprehensive adult and geriatric assessments of hospice patients
  • Evaluate and manage patient plans of care
  • Serve as a proactive, collaborative team member in the hospice care environment
  • Complete all documentation and records in a timely and accurate manner
  • Follow established policies and procedures as related to scope of practice
  • Maintain continuity of patient care in a hospice setting

Required Qualifications

  • Current, active, and unrestricted Registered Nurse (RN) license in state of practice
  • Current, active, and unrestricted Nurse Practitioner (NP) license in state of practice
  • Board certification by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care
