Hospice Licensed Practical Nurse (LPN) - PRN

Interim HealthCare is seeking a compassionate Hospice Licensed Practical Nurse (LPN) to deliver patient-centered end-of-life care in a home-based setting. In this PRN role, you will provide dedicated 1:1 patient care focused on comfort, symptom management, and family support during patients' final transitions.

Clinical Setting

You will float between Greenville and Spartanburg Counties in South Carolina, providing hospice services in patients' homes. This role emphasizes quality care over volume, allowing you to build meaningful relationships with patients and families.

Responsibilities

• Communicate frequently and timely with the registered nurse, supervisor, and other staff regarding patient condition, including participation in case conferences
• Provide accurate and timely documentation of patient services reflecting the plan of care
• Assess and provide patient and family/caregiver education pertinent to diagnosis and self-care planning
• Administer medications and treatments as prescribed by the physician in accordance with nursing standards of practice
• Participate in coordination of hospice services and report identified needs to the interdisciplinary team
• Ensure equipment, supplies, drugs, and other necessary items and services are available
• Provide education and interventions to support effective management of patient pain and physical symptoms
• Assess patient and family response to interventions and treatment

Required Qualifications

• Graduate of an accredited Licensed Practical Nurse (LPN) program
• Active LPN/LVN license in the state in which hospice care is provided
• Minimum one year of nursing experience
