Hospice Licensed Practical Nurse

We are seeking a Licensed Practical Nurse to provide skilled, compassionate care to hospice patients in home and facility settings. This full-time position offers the opportunity to support patients and families through end-of-life care, working closely with an interdisciplinary care team to ensure comfort and dignity.

Schedule

• Full-time, Monday through Friday, day shift

Responsibilities

• Provide direct nursing care to hospice patients in home and facility settings
• Assess patient needs and monitor symptoms, vital signs, and changes in condition
• Administer medications, treatments, and interventions as ordered
• Educate and support patients and families regarding symptom management and end-of-life care
• Communicate changes and concerns promptly to the RN and interdisciplinary care team
• Complete all required documentation accurately and on time

Required Qualifications

• Current LPN license valid for practice in Missouri
• Current driver's license
• Minimum of one year of nursing experience
• Hospice or palliative care experience
• Reliable transportation and valid automobile liability insurance
