Hospice Certified Nursing Assistant

Solace Hospice & Palliative Care Inc is seeking a Hospice Certified Nursing Assistant to provide hands-on personal care and emotional support to patients and families in the Saint Charles, IL area during end-of-life care. This is a per-visit compensation role working in patient homes and care facilities.

Responsibilities

• Provide personal care services including bathing, grooming, oral hygiene, and repositioning for hospice patients
• Assist with activities of daily living (ADLs) in patient homes or care facilities
• Monitor and report changes in patient condition, vital signs, and comfort levels to the nursing team
• Offer emotional support and companionship to patients and their families
• Assist with light housekeeping, meal preparation, and personal laundry related to patient care
• Document patient care activities accurately and in a timely manner
• Collaborate with the interdisciplinary hospice care team to ensure a holistic patient experience

Required Qualifications

• Current CNA certification in the state of Illinois
• Strong understanding of comfort-focused, patient-centered care principles
• Excellent communication and interpersonal skills with patients, families, and care teams
• Reliable transportation and valid driver's license for home visit travel in the Saint Charles, IL area
• Ability to work independently with compassion, patience, and professionalism
• CPR certification
• Ability to meet health screening requirements

Preferred Qualifications

• Prior hospice, palliative care, or end-of-life care experience

Compensation

• Compensation is paid per visit, not per hour
