Hospice Certified Nursing Assistant

Residential Home Health and Hospice is seeking a Certified Nursing Assistant to provide compassionate care to patients in their homes in the Milford, PA area. This is a full-time position working 40 hours per week in a hospice setting focused on comfort, dignity, and quality of life.

Responsibilities

• Deliver patient-centered care in patients' homes, collaborating with an interdisciplinary team
• Provide hands-on assistance with activities of daily living (ADLs)
• Observe and report changes in patient condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan and offering emotional support
• Help patients and families understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness
• Accurately document care provided in the electronic medical record (EMR)
• Travel to patient homes within your assigned territory

Required Qualifications

• Active Certified Nursing Assistant (CNA) certificate in Pennsylvania
• High school diploma or equivalent certificate
• Minimum one year of healthcare experience
• Valid driver's license
• Ability to spend approximately 20% of your workday driving to and from patient locations

• Hospice experience

Schedule

• Full-time, 40 hours per week
• Field-based position requiring travel to patient homes
