Hospice Care Registered Nurse - Field Queens

This position provides nursing care to hospice patients in a field-based setting covering Queens and Westchester. You will conduct comprehensive assessments, develop individualized plans of care, and coordinate with an interdisciplinary team to deliver compassionate end-of-life care in patients' homes.

Responsibilities

• Conduct pre-admission screenings and initial nursing health assessments on new patients, including assessment of body systems (cardiac, vascular, neurological, gastrointestinal, and others)
• Develop nursing plans of care and goals based on individual patient needs
• Provide direct nursing care including supervision of diet, medications, activity levels, and safety in the home environment
• Supervise and support activities of daily living for patients
• Evaluate the provision of nursing care and coordinate services including physical therapy, occupational therapy, speech therapy, nutrition counseling, and respiratory therapy
• Train, evaluate, and supervise home health aides, homemakers, and personal care workers in patients' homes
• Recommend personnel changes to hospice administration as necessary
• Determine the need for and recommend rental or purchase of medical equipment and supplies
• Provide ongoing evaluation of equipment for defects, repairs, or additional needs
• Recommend to the Interdisciplinary Group (IDG) when specialized therapies are warranted based on clinical assessment
• Request physician written orders for changes to the plan of care as appropriate
• Report to and consult with supervising nurses regarding patient status
• Participate in on-call coverage on a rotating basis
• Serve as liaison maintaining patients' plans of care during inpatient stays
• Refer major or recurrent problems to the hospice supervising nurse
• Participate in the ongoing Quality Improvement Program
• Participate in orientation and in-service training offered within and outside the hospice

Physical Demands

• Constant standing and walking
• Frequent stooping, bending, stretching, lifting, pulling, pushing, and turning
• Lifting objects from 10 to 150 pounds
• Pushing objects up to 200 pounds
• Occasional sitting
• Lifting with assistance objects greater than 150 pounds

Required Qualifications

• Graduate from an accredited School of Nursing
• Current license to practice as a Registered Professional Nurse in New York State
• Two or more years of nursing experience (med/surg, hospice, or home care preferred)
• Minimum one year of experience in acute care med/surg (strongly preferred)
• Specialized certifications as needed

Education Requirements

• Bachelor's Degree in Nursing preferred
• Must be enrolled in an accredited BSN program within two years of job entry date
• Must obtain a BSN Degree within five years of job entry date

Preferred Qualifications

• Experience in community health, hospice, and home care
• Med/surg background
• Acute care experience

Travel Requirements

• This is a field position covering Queens and Westchester
