Home Health/Hospice Nurse Practitioner/Practitioner Assistant- Boise (55904)

Terrace Home Health and Hospice is seeking a full-time Nurse Practitioner or Physician Assistant to provide care for home health and hospice patients in the Boise, ID area. You will visit patients in their homes, assisted living facilities, hospitals, long-term care facilities, and group home environments, working independently while collaborating with the home health and hospice team.

Responsibilities

• Perform face-to-face patient encounters
• Collect and assess patient care data
• Develop and implement plans of care
• Manage and evaluate patient care outcomes
• Provide patients and families with health education, counseling, and referrals to other healthcare professionals and community resources
• Participate in case management
• Order treatments and durable medical equipment as indicated

Required Qualifications

• Active Idaho Nurse Practitioner or Physician Assistant license in good standing
• Prescriptive authority
• Reliable transportation
• EMR proficiency

Preferred Qualifications

• Previous home health or hospice experience
