Iowa Home Care is seeking a part-time Registered Nurse to provide specialized wound care services to patients in their homes in the Des Moines area. In this role, you will perform comprehensive wound assessments, develop individualized care plans, and deliver evidence-based wound treatments while collaborating with physicians, patients, families, and the interdisciplinary care team.
Clinical Setting
This is a part-time home health position where you will visit patients in their homes to provide wound management and clinical oversight. You will work with a variety of wound types, including surgical wounds, pressure injuries, diabetic ulcers, vascular wounds, and traumatic injuries.

Responsibilities
- Perform comprehensive nursing assessments and develop individualized plans of care
- Assess, monitor, and treat a variety of wounds including surgical wounds, pressure injuries, diabetic ulcers, vascular wounds, and traumatic injuries
- Perform wound measurements, staging, documentation, and ongoing evaluation of healing progress
- Administer wound care treatments in accordance with physician orders and evidence-based practices
- Educate patients, families, and caregivers regarding wound care, prevention strategies, nutrition, and disease management
- Monitor for signs and symptoms of infection or complications and communicate changes in condition to the physician and care team
- Coordinate care with physicians, therapists, aides, and other healthcare professionals
- Complete Start of Care, Recertification, Resumption of Care, and Discharge assessments as assigned
- Ensure accurate, timely, and compliant documentation within the electronic medical record
- Participate in case conferences and interdisciplinary team meetings
- Maintain compliance with all federal, state, and agency regulations
Required Qualifications
- Current Registered Nurse (RN) license in the State of Iowa
- Minimum one year of nursing experience
- Strong assessment, critical thinking, and clinical documentation skills
- Ability to work independently and manage a flexible schedule
- Valid driver's license, reliable transportation, and current auto insurance
- Current CPR certification
