Home Health Visits Licensed Practical Nurse

BAYADA Home Health is hiring a Licensed Practical Nurse to perform home health visits in the Dartmouth office, serving adult and geriatric clients in Taunton, Raynham, and surrounding towns. This position offers part-time or full-time employment with flexible day schedule options and work/life balance.

Clinical Setting

You will provide home health services to adult and geriatric clients in their homes across designated territories. BAYADA is an accredited, regulated, certified, and licensed home health care provider.

Responsibilities

• Make home visits to clients living in designated territories
• Administer medications, perform wound care, and carry out treatments and procedures as assigned
• Monitor clients' conditions and report changes to the Clinical Manager and Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Provide case management and coordination
• Accurately document observations, interventions, and evaluations using electronic medical records on a state-of-the-art touch pad tablet

Required Qualifications

• Current license as a Licensed Practical Nurse in the state of Massachusetts
• Minimum of one year of recent clinical experience as an LPN
• Ability to travel to cases as assigned

Preferred Qualifications

• Prior home care experience
