Home Health Visiting Registered Nurse

Safe Life Home Health Care, LLC is seeking a Home Health Registered Nurse to provide skilled nursing care to patients in their homes throughout Waukegan, IL and surrounding areas. This full-time or part-time position offers the opportunity to deliver personalized, patient-centered care while helping prevent hospitalizations and supporting patients' health goals in a home setting.

Clinical Setting

You will work with Safe Life Home Health Care, LLC, a leading home health provider committed to delivering compassionate care. The organization utilizes Andy AI to assist clinicians with OASIS completion and submission within 24 hours, and uses WellSky Kinnser EMR for clinical documentation and case management.

Responsibilities

• Conduct comprehensive assessments of patients' health status and needs in their home environment
• Develop and implement individualized care plans based on patient assessments and medical history
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols
• Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition
• Monitor patients' progress, documenting changes in condition and response to treatment
• Provide education to patients and families regarding disease process and management, medication regimen, pain management, fall prevention, infection control, proper diet and exercise, and when to contact pharmacy, physician, or seek emergency care
• Collaborate with interdisciplinary teams to coordinate care, including discharge planning and referrals to other services
• Maintain accurate medical records and documentation in compliance with legal and regulatory standards
• Respond promptly to patient inquiries or concerns, providing support and guidance
• Attend staff meetings, patient care conferences, and in-services as scheduled
• Understand and adhere to established policies and procedures

Required Qualifications

• Valid Illinois RN license
• Valid CPR certification
• Valid driver's license
• Valid auto insurance
• Minimum 1 year of home health experience
• Proficiency in WellSky Kinnser EMR
• Strong clinical assessment and communication skills

Schedule

• 15 to 40 visits per week
• Full-time or part-time positions available
