Home Health Visit Nurse - Full Time, 1st Shift

Bridge Home Health and Hospice is seeking a full-time Registered Nurse to provide direct patient care in the home health and hospice setting. In this role, you will assume ongoing assessment, care planning, documentation, and accountability for assigned patients, utilizing the nursing process to meet each patient's unique physical and emotional needs.

Clinical Setting

You will work within the home health and hospice care system, providing age-specific, disease-specific care to patients served by Bridge Home Health and Hospice in Findlay, Ohio. This position requires you to make visits as assigned by the case manager or scheduler in compliance with each patient's plan of care.

Responsibilities

• Assess, plan, implement, and evaluate patient care using the nursing process
• Provide disease and diagnosis-specific care appropriate to each patient's age
• Establish and maintain communication and collaboration with case managers, the interdisciplinary team, and patients' primary physicians regarding patient condition, orders, plan of care, and anticipated needs
• Make assigned home visits in compliance with the plan of care
• Communicate admission and recertification criteria specific to each patient
• Maintain documentation consistent with or exceeding Bridge Home Health and Hospice guidelines
• Manage weekend assignments and keep case managers and home health/hospice managers informed of any unusual or special incident occurrences
• Utilize a team approach to teaching opportunities and family care issues
• Maintain optimal patient care within budget constraints
• Demonstrate service excellence by practicing the organization's mission, vision, and values to promote patient satisfaction
• Comply with organizational policies, procedures, and practices to ensure quality job performance and Medicare/Medicaid compliance

Required Qualifications

• Current state licensure as a Registered Nurse (RN)
• Minimum of 1-2 years of general nursing experience
• Current Ohio driver's license with a clean driving record
• Ability to provide your own transportation while on duty
• Positive, service-oriented interpersonal and communication skills
• Knowledge and ability to demonstrate care appropriate to the age of patients served, including understanding of growth and development across the life span
• Ability to assess patient data, interpret information, and identify age-specific care requirements

Preferred Qualifications

• Home care or hospice work experience

Physical Demands

This position requires a full range of body motion with intermittent walking, lifting, bending, climbing, squatting, kneeling, twisting, sitting, and standing. You will be required to walk up to 2 hours per day, sit for 4 hours per day, and stand for 2 hours per day. You must be able to lift 50 pounds and reach work above the shoulders. Good eye-hand coordination and fine finger dexterity are required, including the ability to document legibly. Excellent verbal communication skills are essential. Corrected vision and hearing in the normal range are required.

This position is classified at risk for possible occupational exposure to bloodborne pathogens (HBV, HIV, etc.).
