Job Description

Iowa Home Care is seeking a Shift Nurse (RN or LPN) to provide skilled nursing care to patients in their homes throughout the Ottumwa area. This role focuses on delivering direct, hands-on nursing care during scheduled shifts rather than managing multiple patients in a facility setting. You will work consistently with the same patient and family, becoming familiar with their routines, preferences, and individual care needs while helping them remain safe and comfortable at home.

Clinical Setting

You will provide care in patients' homes throughout the Ottumwa area, working one-on-one with assigned patients during your scheduled shifts.

Responsibilities

  • Provide skilled nursing services in the patient's home during scheduled shifts
  • Follow the established plan of care and physician orders
  • Monitor the patient's condition throughout the shift and recognize changes that may require intervention
  • Administer medications and treatments as ordered and within your scope of practice
  • Perform nursing procedures appropriate to the patient's needs and level of licensure
  • Monitor and document vital signs, symptoms, intake and output, and other relevant clinical information
  • Assist with respiratory, nutritional, mobility, and other skilled care needs as applicable to the patient
  • Provide appropriate safety, infection prevention, and emergency interventions
  • Communicate changes in patient condition promptly to the appropriate RN, physician, supervisor, or care team member
  • Maintain accurate and timely nursing documentation
  • Assist patients with maintaining comfort, dignity, and as much independence as possible
  • Provide education and support to family members and caregivers
  • Build a trusting relationship with the patient and family while maintaining professional boundaries
  • Work collaboratively with the patient's broader healthcare team
  • Follow agency policies, clinical standards, and applicable state and federal regulations

Required Qualifications

  • Graduate of an accredited school of nursing
  • One to two years of recent acute care experience in an institutional setting
  • Current RN or LPN licensure
  • Current CPR certification
  • Excellent observation, verbal, and written communication skills
  • Problem-solving skills and basic math skills
  • Nursing skills per competency checklist
  • Valid driver's license with a personally owned automobile in good working order and insured in accordance with organization requirements
