The opportunity

Job Description

Lifespark is seeking an RN Visit Nurse to provide comprehensive home health nursing services to seniors in a regional, multi-city service area. In this role, you will plan, organize, and direct home care services, building on community resources to meet the health needs of individuals and families within their homes. You will combine clinical nursing expertise with community health education to deliver patient-centered care.

Clinical Setting

You will work in home-based settings across multiple cities in a regional assignment, visiting clients in their homes to provide direct nursing care and coordinate services.

Responsibilities

  • Complete initial assessments of clients and families to determine home care needs, including comprehensive physical assessments and health histories
  • Regularly re-evaluate client nursing needs and adjust care plans as client status changes
  • Develop individualized care plans that establish goals based on nursing diagnoses and incorporate therapeutic, preventive, and rehabilitative nursing actions
  • Include clients and families in the care planning process
  • Administer medications and treatments as prescribed by the physician
  • Provide direct client care as defined by the State Nurse Practice Act
  • Counsel clients and families on meeting nursing and related health needs
  • Provide health care instructions appropriate to each client's assessment and care plan
  • Identify and implement discharge planning needs prior to client discharge
  • Act as Case Manager when assigned, coordinating patient care for assigned caseload
  • Establish and maintain professional rapport with clients, clinicians, partners, and the community
  • Ensure positive and professional communication within the team
  • Address customer service concerns and escalate issues as needed
  • Prepare clinical notes and update the primary physician when necessary and at least every 60 days
  • Communicate with the physician regarding client needs and report any changes in condition
  • Coordinate care with community health-related professionals
  • Complete all documentation related to the care plan by 10 a.m. the next day
  • Comply with all rules and regulations governing registered nurse responsibilities
  • Complete orientation and annual training requirements
  • Participate in Lifespark's Event Management process, including initiating reports and investigations
  • Promote workplace safety through real-time coaching and participation in safety awareness training
  • Instruct, supervise, and evaluate home health aide care every two weeks
  • Manage client supplies and ensure appropriate DME provider utilization
  • Schedule and maintain member appointments
  • Organize travel efficiently and document mileage accurately within the EMR
  • Perform other duties as assigned

Required Qualifications

  • One to two years of recent acute care experience in an institutional setting
  • Graduate of an accredited school of nursing
  • Current Minnesota Nursing License
  • Pass a Motor Vehicle Record (MVR) check per Lifespark company policy
  • Proof of COVID-19 vaccination or documentation of exemption in accordance with CMS Vaccine Mandate

Preferred Qualifications

  • One year of home health care experience
  • Experience in customer service or direct member contact
  • Experience supervising caregivers

Schedule

  • Full-time position (1.0 FTE)
  • Monday through Friday, 8:00 a.m. to 5:00 p.m.
  • Every 6th weekend rotation required

Compensation and Benefits

  • Salary: $85,000 to $87,000 per year, depending on experience
  • Mileage reimbursement at $0.70 per mile
