Home Health RN - Registered Nurse

Home Care Solutions is seeking a Registered Nurse to provide and direct nursing care to patients in their homes throughout the Mt. Juliet, Tennessee area and surrounding communities. This full-time position offers the opportunity to work with diverse patient populations while maintaining strong connections with individuals and families across the region.

Responsibilities

• Provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies
• Coordinate the total plan of care with other healthcare professionals involved in patient care
• Maintain continuity of patient care by planning and exchanging information with physicians, agency personnel, patients, families, and community resources
• Make initial and comprehensive nursing evaluation visits
• Determine the primary focus of care and develop plans of care within state-specific guidelines
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer
• Maintain and submit accurate documentation and clinical notes, including patient condition, appropriate goals, interventions, and patient response to home health care
• Provide high-quality clinical services within the scope of practice as defined by state nursing laws, from admit through discharge

Required Qualifications

• One year of clinical RN experience
• Current RN license in the state where services are rendered
• Current CPR certification
• Valid driver's license
• Vehicle insurance
• Access to a dependable vehicle or public transportation
• Ability to reliably commute to multiple patient residences daily within the assigned geographic area

Work Environment

• On the road; position requires travel to multiple patient homes daily
