Stonerise Home Health is seeking a Home Health RN Field Nurse to provide skilled nursing care to patients in their homes. In this role, you will conduct initial patient assessments, develop and implement individualized care plans, supervise clinical staff, and coordinate care across an interdisciplinary team. You will work independently in the community, managing patient caseloads and serving as a case manager when assigned. This position offers the opportunity to make a meaningful impact on patients' lives while supporting their independence and dignity in their own homes.
Clinical Setting
You will provide home-based nursing care to patients throughout the community, working from the office located at 30 Mon General Drive, Suite 2, Morgantown, WV 26505.
Responsibilities
- Conduct comprehensive initial assessments of patients and families to determine home care needs, including complete physical assessments and medical history
- Develop, implement, and modify individualized plans of care based on nursing diagnoses and patient status
- Supervise and direct Licensed Practical Nurses (LPNs) and Home Health Aides in implementing patient care plans
- Develop and direct patient education plans as needed
- Administer medications and treatments as prescribed by the physician
- Evaluate and revise the plan of care as patient needs and status change
- Identify and implement discharge planning needs prior to patient discharge
- Act as Case Manager when assigned, coordinating care for assigned caseloads
- Prepare clinical notes and communicate with physicians regarding patient needs
- Provide supervision of clinical staff via patient feedback and onsite interactions
Required Qualifications
- Graduate of an accredited school of nursing
- Active RN licensure
- One to two years of recent acute care experience in an institutional setting
- Valid driver's license with a personally owned, insured automobile in good working order
Preferred Qualifications
- Bachelor's degree in nursing
- One year of home health care experience

Additional Requirements
You must maintain a valid driver's license and provide your own insured automobile that meets state and organizational requirements and is in good working order. You will be required to travel to patients' homes throughout the service area.
