Home Health Registered Nurse

Well Care Health is seeking a Registered Nurse to provide compassionate, individualized care to patients in their homes. In this role, you will assess and monitor patients' health conditions, develop and implement personalized care plans, educate patients and families on health management and self-care, and collaborate with a multidisciplinary team to ensure optimal patient outcomes. You will work with patients across all developmental stages, including adults (18-72 years) and geriatric patients (72+ years).

Responsibilities

• Assess, interpret, plan, implement, and evaluate patient care according to the patient's age and diagnosis
• Effectively and efficiently manage a caseload of patients and coordinate care with a multidisciplinary team
• Communicate information appropriately in accordance with agency policies, procedures, and discipline-specific guidelines
• Organize and perform work effectively and efficiently
• Maintain and adjust schedule to enhance agency performance
• Maintain and promote customer satisfaction
• Demonstrate the mission, vision, and values of the agency daily
• Demonstrate positive interpersonal relations with all members of the agency

Required Qualifications

• Graduate of an accredited or approved school of nursing (AD, Diploma, or BSN program)
• Current license to practice professional nursing in North Carolina or South Carolina
• CPR certification
• One year of RN experience and a total of 2 or more years of clinical experience
• Valid North Carolina driver's license and an operational vehicle
• Hand/eye coordination to give injections, use computer, and manipulate patient care equipment
• Ability to communicate and be literate in English
• Ability to transfer and/or maneuver objects weighing at least 50 pounds
• Ability to assess a patient's condition, formulate a plan of care, select appropriate interventions, and evaluate patient response to care
• Ability to visually assess patients and utilize sight to implement and evaluate plan of care
• Ability to utilize hearing to auscultate lung sounds and bowel sounds, hear alarms, and communicate effectively

Preferred Qualifications

• Home health experience

Education

• Graduate of an accredited or approved school of nursing (AD, Diploma, or BSN program)

Licenses and Certifications

• Current RN license in North Carolina or South Carolina
• CPR certification

Experience

• One year of RN experience required
• Total of 2 or more years of clinical experience required
• Supplemental clinical experience may include LPN, CNA, military medic, EMT, or related experience

Schedule

• Variable Monday through Friday, weekends and holidays as needed
• Flexible schedule to accommodate staffing needs

Physical Requirements

• Frequent pushing, moving, and lifting of patients
• Positioning of patients, giving patients baths, and ambulating patients requiring significant physical effort
• Occasional overhead reaching, stair climbing, and fine motor manipulation
• Exposure to noise, bloodborne pathogens, body fluids, infectious diseases, needle puncture wounds, dangerous animals, traffic hazards, temperature changes, and weather extremes

Compensation and Benefits

• Salary range: $74,880-$81,883.72
