Compassus is seeking a Home Health Registered Nurse - Triage PRN to provide after-hours triage and support for home health and hospice patients and field staff. This remote position involves accepting and facilitating new referrals, providing community education, collaborating with physicians, and assisting with patient scheduling and telehealth monitoring.
Responsibilities
- Accept and facilitate new referrals for home health and hospice patients, both in-system and out-of-system
- Triage all patient and facility calls after hours
- Act as a liaison to provide community education for referral sources, patients, and families
- Provide accurate and responsive service to all calls received during weekend hours
- Collaborate with physicians and obtain orders as needed to facilitate patient care
- Provide guidance and support to field staff during hours worked
- Assist in scheduling patient admissions, routine visits, and PRN visits for nurses, therapists, and home health aides
- Monitor telehealth as needed
- Perform other duties as assigned
Schedule
- Every Saturday and Sunday, 9:00 AM to 7:00 PM CST
Required Qualifications
- Associate or Bachelor's degree in Nursing
- Two years of full-time experience as a Registered Nurse
- Active and unencumbered Multi-State Registered Nurse license in state(s) of employment
- Alaska, California, and Michigan RN licensure required; must be willing and able to obtain additional state licensures if required
- Current CPR certification
Preferred Qualifications
- One year of experience as a registered nurse in a clinical care setting or home health
- Home health experience

- Mathematical skills including ability to add, subtract, multiply, divide, and compute rate, ratio, and percentage
- Language skills including ability to read, analyze, and interpret business periodicals, professional journals, technical procedures, and governmental regulations; ability to write reports and business correspondence; ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communication skills
- Ability to understand, read, write, and speak English
- Ability to articulate and embrace integrated healthcare at home philosophy
Physical Demands and Work Environment
- Regular standing, walking, and manual dexterity
- Ability to lift and move objects up to 50 pounds
- Visual acuity requirements including close and distance vision, color and peripheral vision, depth perception, and ability to adjust focus
- Exposure to bodily fluids, infectious diseases, and conditions typical to healthcare settings
- Routine use of standard medical equipment and tools associated with clinical care
