Compassus is seeking a Home Health Registered Nurse - Triage to provide after-hours support for home health and hospice patients and field staff. This is a remote position working weekends only.
In this role, you will triage all patient and facility calls during after-hours weekend coverage, facilitate new and existing home health referrals both within and outside the system, and provide clinical support to field staff. You will act as a liaison to referral sources, patients, and families, providing accurate and responsive service to all calls received.
Responsibilities
- Accept and facilitate new referrals for home health and hospice patients from in-system and out-of-system sources
- Act as a liaison to provide community education for referral sources, patients, and families
- Provide accurate and responsive service to all calls received during weekend hours
- Collaborate with physicians and obtain orders as needed to facilitate patient care
- Provide guidance and support to field staff (nurses, therapists, and home health aides) during hours worked
- Assist in scheduling patient admissions, routine visits, and PRN visits for field staff
- Monitor telehealth as needed
- Perform other duties as assigned
Schedule
- Every Saturday and Sunday, 9:00 AM–7:00 PM Central Time
- Remote position
Required Qualifications
- Associate or Bachelor's degree in Nursing
- Active and unencumbered Compact Registered Nurse license in state(s) of employment
- CA, OR, MI RN licensure required; able and willing to obtain additional licensures
- Current CPR certification
- One year of experience as a registered nurse in a clinical care setting or home health preferred
State-Specific Requirements
- Louisiana: Two years of full-time experience as a Registered Nurse or two years of clinical experience in home health required

Physical Demands and Work Environment
- Regular standing, walking, and manual dexterity
- Ability to lift and move objects up to 50 pounds
- Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and ability to adjust focus
- Exposure to bodily fluids, infectious diseases, and conditions typical to healthcare settings
- Routine use of standard medical equipment and tools associated with clinical care
