Home Care Providers of Texas is seeking a Full-Time Registered Nurse to deliver skilled nursing care to patients in their homes in the San Antonio, TX area. This role focuses on geriatric home health care, with over 90% of your time spent on direct patient care rather than administrative tasks. You will use the Roger app, an AI-driven, voice-enabled, HIPAA-compliant documentation platform designed to reduce charting time by 50% while maintaining OASIS compliance.
Clinical Setting
You will provide care in patients' homes as part of a forward-thinking home health care team. The role emphasizes flexibility, independence, and support for your schedule.
Responsibilities
- Deliver high-quality, comprehensive skilled nursing care in accordance with physician orders and individualized plans of care
- Partner with the interdisciplinary care team to develop, implement, and update personalized care plans addressing each patient's unique needs
- Administer nursing treatments and manage clinical activities to support optimal health outcomes
- Continuously assess patient progress and the effectiveness of services
- Communicate findings with the care team, patient, family, physician, and case manager
- Perform additional duties as needed
Required Qualifications
- Active RN License (Texas or Compact)
- Active CPR Certificate
- Prior experience in geriatric home health care
- Valid Driver's License
- Self-starter with strong organizational skills
